Sara Leonard, MD, FAAFP is the Geriatrics Fellowship Director and Director of Service Innovation for the Parker Division of Geriatrics in the Department of Family Medicine and Community Health at Rutgers Robert Wood Johnson Medical School in New Brunswick, NJ. She is the 2024-2025 President of the New Jersey Academy of Family Physicians.
Dr. Leonard graduated with honors from AUA College of Medicine and completed her Family Medicine residency and Geriatrics fellowship at the Rutgers-RWJ programs at CentraState Medical Center in Freehold, NJ. She holds a CAQ in Geriatric Medicine. Prior to joining the faculty at Rutgers-RWJ she was the medical director of a large house call practice in the Parker Advanced Care Institute at VNA Health Group. Dr. Leonard has been on the NJAFP Board continuously since 2011, beginning her board service as a Resident Trustee.
