NJAFP is working with the New Jersey Department of Health to promote Diabetes Prevention Programs (DPP) and Diabetes Self-Management Education (DSME).  This initiative will help implement sustainable changes in primary care practices that will result in improved outcomes for patients with diabetes.

Evidence shows that diabetes self-management education can lower A1c (ADA, 2015) by teaching patients to better manage food, exercise and medications.  Find your local DPP, DSME and DSMP programs at https://drive.google.com/open?id=1ES-_UwMwqavmH1JKYV-lS_syRFg&usp=sharing.

When to refer patients:

  • With a new diagnosis of type 2 diabetes.
  • Annually for health maintenance and preventing complications.
  • When new complicating factors influence self-management.
  • When transitions in care occur.
