Implement the ABCs of Diabetes in Your Practice

NJAFP is launching a quality improvement and education program on the "ABCs of Diabetes." This initiative will use a learning collaborative model to implement sustainable changes in primary care practices that will result in improved outcomes for patients with T2DM through:

  • identification of barriers to medication use and implementation of strategies to break down those barriers;
  • use of patient-engagement resources; and
  • implementation of self-management plans along with referral to formal diabetes self-management education programs to control A1C, blood pressure, LDL, and support smoking cessation.

Physicians enrolled in this program will earn CME/MOC credit and engage with experts from across the state.
